Clinical Evidence: Lean Mass Preservation Across 12–24 Month Trials

The longest-duration trial examining MK-677 for age related muscle loss was a 2008 randomised, double-blind, placebo-controlled study published in the Annals of Internal Medicine. The trial enrolled 65 adults aged 60–81 and administered 25mg MK-677 daily for 24 months. The primary endpoint was change in lean body mass measured by DEXA scan. At 12 months, the MK-677 group showed a mean increase of 1.1kg lean mass versus baseline, while the placebo group lost 0.5kg. At 24 months, the MK-677 group maintained baseline lean mass (+0.2kg from month 0), while placebo participants lost 2.1kg. A statistically significant difference that persisted across the full trial duration. Secondary outcomes included grip strength (no significant difference), gait speed (modest improvement in MK-677 group at 18 months but not sustained at 24 months), and falls (no significant difference). These functional outcomes are important context: MK-677 preserved lean mass but did not translate to dramatic improvements in physical performance metrics. This is consistent with what we've observed in similar datasets. Hormonal restoration prevents further decline but does not reverse neuromuscular deconditioning that's already established. Lean mass preservation is necessary but not sufficient for functional recovery in older adults who have already experienced significant sarcopenia. Another key finding: IGF-1 levels in the MK-677 group averaged 220–240 ng/mL throughout the trial, compared to 140–160 ng/mL in placebo. This sustained elevation is the mechanism behind the lean mass effect. It shifts the balance between protein synthesis and degradation just enough to offset age-related catabolic pressure. The trial also measured bone density and found no significant change in either group, which is notable because GH is often promoted for bone health. MK-677's effect appears specific to lean tissue preservation, not broad anabolic effects across all tissue types. Adverse events were mild and primarily GI-related: transient nausea in 12% of participants during the first two weeks, and mild lower extremity oedema in 8% that resolved without dose adjustment. Fasting glucose increased slightly (+4 mg/dL on average) but remained within normal range. No participants developed diabetes or required discontinuation due to metabolic side effects. This safety profile is one of the reasons MK-677 continues to appear in gerontology research. It's well-tolerated in populations where polypharmacy and metabolic comorbidities are the norm.
