- Temperature excursion
- Protein denaturation above 8°C irreversibly destroys tertiary structure required for receptor binding
- Check refrigerator thermometer. If vial was ever left at room temp >2 hours, assume compromised
- Discard affected vial; store all future reconstituted peptides at 2–8°C continuously
- Invisible failure. Denatured peptides look identical to functional ones but have zero activity
- Injection timing misalignment
- Injecting during low endogenous GH periods wastes the exogenous pulse; insulin elevation from recent meals suppresses GH release
- Review injection log. If doses administered <3 hours post-meal or mid-afternoon, timing is suboptimal
- Inject either 30 min pre-bed or first thing upon waking, minimum 3 hours fasted

- Contamination from poor sterile technique
