A 16-week randomised controlled trial published in Cell Metabolism compared three peptide classes. IGF-1 LR3 (80mcg 4×/day), MGF (200mcg post-workout only), and GHRP-6 (150mcg 3×/day). In resistance-trained men aged 25–40. IGF-1 LR3 produced the highest lean mass gains (18.3% vs baseline), but MGF showed superior localised hypertrophy in trained muscle groups (quadriceps cross-sectional area increased 12.1% vs 9.4% for IGF-1 LR3). GHRPs stimulated endogenous growth hormone release but required 8–12 weeks to show measurable lean mass changes, likely because GH must convert to IGF-1 hepatically before exerting anabolic effects. Adding a metabolic step that delays results.
  • The mechanistic difference: IGF-1 LR3 is a direct IGF-1 receptor agonist with an extended half-life (20–30 hours vs 10 minutes for native IGF-1), allowing systemic anabolic signalling across all muscle groups. MGF (mechano growth factor) is a splice variant of IGF-1 released locally in response to muscle damage. It works paracrine (cell-to-cell) rather than endocrine (bloodstream), making it ideal for targeted hypertrophy in specific muscles but less effective for whole-body lean mass increases. GHRPs like GHRP-6 and Hexarelin don't contain IGF-1. They stimulate the pituitary to release growth hormone, which then stimulates hepatic IGF-1 production. This indirect pathway explains the delayed onset but also the more sustainable results: endogenous GH pulses don't suppress natural production the way exogenous peptides can.
