- First-line option. Faster onset, lower dose
- Viable alternative if injections aren't feasible
- 2–3 weeks for noticeable improvement
- 8–10 weeks on endoscopy
- Subcutaneous preferred for speed; oral acceptable if compliance with higher dosing is reliable
- Moderate Crohn's (CDAI 220–450)
- Preferred. Higher bioavailability critical for inflammatory burden
- Requires higher doses and patience. Slower onset
- 3–4 weeks for partial response, 6–8 weeks for significant improvement
- 10–12 weeks minimum
- Subcutaneous strongly recommended. Oral may underdose at standard ranges
- Severe Crohn's (CDAI >450)
- Adjunct only. Biologics or immunosuppressants remain primary therapy
- Not recommended as monotherapy
- Variable. Often used alongside biologics to reduce steroid dependence
- 12+ weeks if combined with standard therapy
- KPV is not a replacement for biologics in severe disease. It's a complementary anti-inflammatory tool
