- The following table compares PT-141 central nervous system sexual arousal to other interventions used in sexual dysfunction research. Understanding where each compound acts. And what type of dysfunction it addresses. Is critical for experimental design.
- PT-141 (Bremelanotide)
- Melanocortin receptor agonist (MC3R/MC4R)
- Hypothalamus, paraventricular nucleus
- 45–60 minutes
- Central arousal deficits (HSDD, AISD)
- First-line for desire disorders unresponsive to vascular treatments
- Sildenafil (Viagra)
- PDE5 inhibitor
- Vascular smooth muscle (genital)
- 30–60 minutes
- Vascular erectile dysfunction
- Ineffective for low libido. Treats blood flow, not desire
- Flibanserin (Addyi)
- 5-HT1A agonist / 5-HT2A antagonist
- Prefrontal cortex, serotonin receptors
- Chronic (daily dosing, weeks to effect)
- HSDD in premenopausal women
- Requires daily dosing and alcohol restriction; modest efficacy vs placebo
- Testosterone (transdermal)
- Androgen receptor agonist
- Systemic (hypothalamus, muscle, bone)
- Weeks to steady-state
- Hypogonadism-related low libido
- Effective when testosterone deficiency is documented; not a CNS arousal agent
- Oxytocin (intranasal)
- Oxytocin receptor agonist
- Limbic system, hypothalamus
- 15–30 minutes
- Social bonding, genital sensation
- Enhances existing arousal; doesn't initiate desire independently
- PT-141 central nervous system sexual arousal stands out because it initiates desire centrally without requiring daily dosing or chronic administration. Flibanserin (Addyi), the only other FDA-approved treatment for female HSDD, requires daily oral dosing for at least 4–8 weeks before effects are observed, and it carries a black-box warning for severe hypotension when combined with alcohol. PT-141 is administered subcutaneously as-needed, typically 45 minutes before anticipated sexual activity, and produces measurable desire effects within one hour.
