  • Use PT-141 for central desire dysfunction; PDE5 inhibitors for vascular ED. The mechanisms do not overlap. One is not a substitute for the other.
  • The bottom line from a mechanism standpoint: PT-141 and PDE5 inhibitors are not interchangeable. A patient with low desire but normal vascular function won't benefit from sildenafil. A patient with severe vascular ED but normal central arousal won't achieve sufficient mechanical function from PT-141 alone. The decision between them depends entirely on whether the dysfunction is central (arousal, motivation, desire) or peripheral (blood flow, vasocongestion). In our experience working with peptide researchers, the most common misuse of PT-141 is administering it to patients with purely vascular ED. Where the expected outcome is disappointment.
