A drug’s benefit is only realized by patients who keep taking it, and the tolerability-driven dropout tempers the practical value of the statistical efficacy.3,10 Second, the entire high-quality evidence base is confined to a single, specific population: premenopausal women with acquired, generalized HSDD. The trials were not designed to, and do not, establish efficacy in postmenopausal women, in women with situational or lifelong low desire, or in men. Extrapolating the RECONNECT results beyond their studied population is not supported by this evidence. The bottom line on the clinical evidence is therefore nuanced rather than triumphant. PT-141 has a level of human evidence that most research peptides can only dream of: two positive, pre-registered, placebo-controlled Phase 3 trials and an FDA approval built on them. But that evidence supports a modest benefit in a narrow indication, achieved at the cost of a demanding side-effect burden. Calling this a “revolution” is defensible only in the mechanistic sense of being first-in-class; it is not defensible as a claim of large or transformative clinical efficacy.
