It was the first GHRH analogue to receive FDA approval (for paediatric GH deficiency, 1997), though it was subsequently withdrawn from the U.S. market for commercial rather than safety reasons. Structurally, sermorelin differs from tesamorelin in that it lacks the full 44-amino acid sequence and the stabilising fatty acid modification. Tesamorelin’s greater receptor binding affinity and longer functional duration in plasma give it a more potent and sustained effect compared with sermorelin at equivalent doses. The clinical evidence base for tesamorelin in adult metabolic indications substantially exceeds that for sermorelin, which has been studied primarily in smaller Phase 2 trials in older adult and anti-ageing medicine contexts.
